Evidence hierarchy 1. Controlled approval/decision record 2. Primary operational record 3. Independent/reviewer verification 4. Supporting documentary/media evidence 5. Reconstructed historical evidence, explicitly labelled Quality principles No silent correction of controlled records; no inflation of proposed work into completed outcomes; distinguish participation from competency; preserve superseded versions; use minimum necessary personal data; investigate credible safety/ethics/privacy/integrity concerns; close actions with evidence. Review levels Activity review · Programme review · Site review · Domain review · Annual institutional review. Escalation Material safety, ethics, privacy, research-integrity, certificate-integrity or governance concerns may trigger immediate restriction pending authorised review.
